Job Description - Certified Diabetes Educations, Health Educators, Pharmacists:

  1. Review and analyze the medical and pharmacy claims data
  2. Stratify patients into risk categories based on severity of asthma symptoms and utilization of medical services.
  3. Attempt to contact patient with chronic diseases (Asthma, Diabetes) to provide an informational flyer reviewing the medication therapy program, patient education brochure and DVD.
  4. Telephonically contact all identified patients using claims data from both medical and pharmacy services.
  5. erform a complete medication history and review. 
  6. Scheduled all patients to attend the Healing Our Village, Inc. Wellness on Demand™ programs. 
  7. Provide consultation to all patients on multiple medications and/or have been identified as non-adherent with their treatment regimen.
  8. nitiate and/or review an medication action plan. 
  9. Identify all inappropriate medication utilization patterns and recommendations for drug therapy changes.  All changes will be communicated by the health educator/ clinical pharmacist to the primary care provider via a written consult which will be emailed, faxed or mailed to the provider. 
  10. Provide an evaluation and report of all clinical and economic outcomes.
  11. Providing diabetes education with a focus on blood glucose monitoring teaching.  Conducts health education presentations with small groups and/or individual counseling sessions regarding diabetes self-management and healthy lifestyles. Educational sessions must encompass all topics required/recommended by the American Diabetes Association for recognition as an ADA Certified Diabetes Education Program and the American Heart Association.
  12. Provide suggestions for the modification of a core health education curriculum
  13. Maintains patient education records, completing all required documentation related to health education classes including sign-in sheets, pre/post tests, member satisfaction surveys.

Job Description

  1. Contact patient to set up interviews either telephonically or via home vists.
  2. Facilitate enrollment into the Wellness Connexions™ program. 
  3. Provide training in the appropriate use of self-monitoring tools (blood glucose monitors, blood pressure cuffs, A1C testing kits, etc.)
  4. Work with the care managers at the health plan to order medical supplies, facilitate referrals to health plan clinical specialty network (i.e. podiatrists and ophthalmologists as necessary)
  5. Perform chart review/placement of diabetes flow sheets and documentation of HEDIS measures
