Advanced Pharmacist Training in Diabetes Management: Focus on Medication Therapy Management

12-16 hours of advanced training in diabetes management.  You will receive a review of the latest evidence based treatment guidelines and  recommendations for the management of diabetes.  Focus will be placed on the appropriate use of oral and injectable medications to achieve glycemic and cardiovascular targets.  Participants will also receive hands on demonstrations of insulin pumps, pens, and blood glucose monitors.  Emphasis will be placed on strategic marketing and advertising of services.

Goals:

  1. To provide an interactive, culturally-sensitive, comprehensive training program designed to equip pharmacists with the tools required to provide medication management services to a broad range of patients with diabetes.  Pharmacists can immediately begin to implement strategies required to effectively interact with patients and healthcare providers as an expert in the area of diabetes to suggest intensification of the medication regimen and where appropriate provide recommendations to the Primary Care Providers (PCPs) on the optimal timing for conversion to insulin therapy.

  2. To create Pharmacist Based Diabetes Care Centers and Inpatient Diabetes Education Programs throughout the United States focused on improving diabetes management nationwide. 

At the end of the program the pharmacist should be able to:

  1. Provide medication therapy counseling and education to patients with diabetes, hypertension, coronary heart disease and obesity

  2. Initiate and adjust insulin therapy and oral medications in type 1 and type 2 patients with diabetes as part of a team management strategy

  3. Provide a medical consultation to healthcare providers recommending modification in current drug therapy to achieve clinical targets.

  4. Set-up a diabetes care center in a inpatient or outpatient practice setting

  5. Understand and implement the documentation and record keeping necessary for diabetes medication therapy management interventions

  6. Develop a strategic marketing and advertising plan for a Diabetes Self -Management Training/Education program

  • Overview of diabetes management with focus on ACCORD, ADVANCE, VADT and ADA, EASD and Texas algorithms
  • Case based approach to the selection and adjustment of oral antidiabetic medications
  • Case based approach to the management of cardiovascular disease and obesity
    Physical Assessment techniques - diabetes, obesity, cardiovascular disease, neuropathy, nephropathy and retinopathy
  • Communication strategies in the management of culturally diverse populations with diabetes
  • Case based approach to insulin dosing including both compensatory and anticipatory insulin therapy
  • Case-based approach to the management of Inpatient Diabetes including implementation of standing order sets and bedside diabetes education
  • Key concepts in diabetes patient behavior modification – Motivational Interviewing
  • Strategic marketing and promotional activities required to building a client base
  • Improving communication skills with patients and other healthcare providers – Role Playing Exercises
  • New technologies in the management of type 1 and type 2 diabetes: focus on the monitoring of blood glucose, hemoglobin A1C, and cholesterol, insulin pump and injection devices, continuous blood glucose monitoring, (meters and A1C), Medtronic – Insulin pump and continuous monitoring- Insulin pens
    Documentation requirement for clinical activities and consultation notes with primary care providers
