Healing Our Village, Inc. has designed a simple to use outcomes tracking tool to assist pharmacists, healthcare providers, insurance companies, non-profit organizations and patients with the information required to improve management of chronic diseases. With a focus on chronic diseases (diabetes, obesity, cardiovascular disease, asthma, etc) the searchable patient database provides basic demographic information along with information on patient care practices and self-care techniques.
WellTRAK™ has been created to function within a health care system and/or a community based outreach program.

Community Based Programs
Many non-profit organizations are involved in chronic disease prevention and awareness programs in the areas of Diabetes, Cardiovascular Disease, Breast and Prostate Cancer, Asthma, and HIV/AIDS. As part of their community outreach efforts health screenings and educational workshops are provided as part of a collaborative effort with many community based partners. One of the many challenges in the area of health promotion is the ability to track health outcomes and improve-ment in overall healthcare. WellTrak™ has been designed to help with the tracking of specialized programs that are developed by the nonprofit organization and determination of site specific outcome measures that can be used to measure the success of a program. Aggregate reports can be generated back to the Sponsoring organization to assist in determining overall program evaluation and effectiveness.
